A 70 year old male patient labourer resident of Miryalguda  by occupation came with complains of fever , burning micturition , pain during micturition along with stomach pain and decreased urine output. 

HOPI - 

H/o Fever since 1 week 

H/o burning and Pain during micturition since 5 days 

H/o stomach pain since 5 days 

H/o decreased urine output since 5 days 

Patient also has weakness. 

No H/o of Blood in urine 

                 Increased urination

                 Increased frequency of micturition 

                 Bed wetting  

                 Slow stream , hesitancy and terminal dribbling 


PAST HISTORY 

Not  a known case of Hypertension , Diabetes Mellitus , Asthma , Epilepsy, CAD . 

Patient had H/o TB 20 years back which lasted for 6 months . He took treatment in Miryalguda . And has no associated symptoms. 


FAMILY HISTORY 

Not significant 


PERSONAL HISTORY 

Diet - mixed 

Appetite- adequate 

Bowel and bladder- not regular 

                                 Pain and burning during micturition 

Addictions- Bidi  (3-4 ) In a day 

                    Alcohol- weekly once  


GENERAL EXAMINATION 

Patient is conscious, coherent and cooperative 

No Pallor, Icterus , Cyanosis, Clubbing , lymphadenopathy, Edema. 







VITALS 

Temperature- Afebrile 

BP- 80/60 mmHg 

PR - 72 bpm

RR- 14 cpm 

SpO2 - 96%


SYSTEMIC EXAMINATION

A 60 year old male with quadriparesis

  This is an online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent.  13/08/2022 Blog by Janhavi Virani  Roll no 58  A 60 year old male patient resident of Yadadri who is a farmer by occupation came with complaints of -  1. Burning micturition since 4 months  2.  Abdominal distension since 2-3 months  3.  Upper and lower limbs swelling since 20 days   4. constipation since 4 days HOPI -  Patient was apparently asymptomatic 11 years back when he got some injury to his right knee followed by which he developed swelling ( hematoma ) and went through operation.  4 and half year back he fell down had a injury to the lower back region following which he had weakness of  all four limbs . For 2 months he took conservative management ( eating fish and eggs ) regained power gradually and was able to walk initially and later was able do some of his daily chores . Pedal Edema since 3 months  2. Shortness of breath since 3 months  HOPI -  Patient was apparently asymptomatic 12 years back .  He was having complaints of vomitings and chest pain and was diagnosed with Ischaemic Heart disease and went through bypass surgery (CABG) .  The same time he was diagnosed with DM and Hypertension.  3 months back was having pedal Edema which is pitting type and extends till knees . shortness of breath on walking a long distance for which he went to hospital and was diagnosed with kidney disease.  He came here 4 days back for dialysis. He had 2 rounds of dialysis till now .  DAILY ROUTINE-  PAST HISTORY-  A known case of Diabetes
